This feature allows up to six files to be attached to a Facility Use Request such as permits and signed certifications, as shown below.


Important: Insurance Certificates are uploaded with your Group Profile not with the submitted Request described here. See Upload Group Insurance information.

To upload a file with the Facility Request:

1. Select the Choose File button in the first Attachment field to display a file navigation screen.

2. Navigate to the desired file, select it and select the OK button.

Once a file is uploaded, its file name will appear to the right of the Choose File button.

3. Enter a description or note related to the attachment in the free-text fields under the first file name.

Note: It is recommended that any required Attachments for a specific space be listed in the Notes field of the Manage Space screen so Group Managers provide the correct files with their Requests.

4. Repeat steps 1 – 3 for any additional files that need to be uploaded with this Request.
